Year 6 Leaver's Hoodie - order form

This year the PTA will be organising for your child to have a Leavers Hoodie in addition to the Leavers Yearbook. Attached is a picture of the colour of the hoodie which will be black with the Monkspath Logo in gold. The children’s signatures ( first name only) will be on the back and these will also be in gold.

So that the PTA can order the correct size for your child please indicate which size you would like to order together with the quantity you wish to order. Different prices apply according to size measurements, as below.
