• Request a Leave of Absence during term time (to be completed at least 4 weeks in advance)
• Use of emergency salbutamol inhaler
• Consent to administer emergency epipen

• Consent for child to walk to school unaccompanied: Year 5
• Consent for child to walk to school unaccompanied: Year 6
