Year 6 Leaver's Hoodie - child's name to be included consent

Dear Parent/Carer

This year the PTA will be organising for your child to have a Leavers Hoodie in addition to the Leavers Yearbook. Attached is a picture of the colour of the hoodie which will be black with the Monkspath Logo in gold. The children’s signatures ( first name only) will be on the back and these will also be in gold.

In order to have your child’s signature on the back you must give permission by completing the form below.

If we do not have your permission, we cannot add the signature to the hoodie. Consent for signatures must be received before, no later than 3:30pm on Friday 11th April .
